Description

Telmisartan is an antihypertensive agent and an angiotensin II receptor antagonist (type AT1). It exhibits a high affinity for the AT1 receptor subtype, displacing angiotensin II and reducing plasma aldosterone levels. Telmisartan specifically affects the renin-angiotensin system without impacting other receptor subtypes or ion channels. It does not inhibit ACE (kininase II) and does not have side effects associated with bradykinin.

The use of telmisartan is of interest to athletes as it reduces left ventricular hypertrophy (LVH) and visceral fat. LVH, a response to intense weight training, can be amplified by steroid use. Reductions in visceral fat are associated with decreased cardiovascular risk. Telmisartan serves as a harm-reduction strategy, lowering overall mortality and risks of cardiovascular diseases.

Pharmacokinetics

When taken orally, telmisartan is rapidly absorbed from the gastrointestinal tract with a bioavailability of 50%. How to use

For adults, the recommended daily dose is 20-40 mg once a day, with some patients responding well to 20 mg/day. The dose can be increased to 80 mg/day if necessary. Dose adjustments are not required for patients with impaired renal function or elderly patients. Patients with impaired liver function should take a daily dose of 40 mg.

Athletes can use daily doses of 20 to 40 mg to reduce the risk of atherosclerosis, cardiovascular disease, and stroke in steroid-using bodybuilders with prehypertension. Telmisartan can also improve HDL cholesterol levels, insulin sensitivity, mitochondrial activity, endothelial function, and cognitive function.
